This data contains a list of California Licensing and Certification District Offices. The California Department of Public Health, Center for Health Care Quality, Licensing and Certification (L&C) Program licenses and/or certifies more than 30 types of healthcare facilities in California. In addition, the Program contracts with the Los Angeles County (LA) to provide federal and state survey and certification services throughout that area; LA is divided into 4 District Offices. The L&C District Office contact information is geocoded. The L&C Program is mandated to conduct various state re-licensing surveys to ensure that a provider is in compliance with all state laws and regulations by conducting on-site inspections (surveys) and investigating complaints and facility-reported events. The L&C Program is also contracted by the United States Department of Health and Human Services Centers for Medicare and Medicaid Services (CMS) to ensure that facilities accepting Medicare and Medi-Cal payments meet federal requirements. L&C is the largest program within CDPH, consisting of about 1,200 managers and staff located at headquarters in Sacramento and 14 district offices throughout the state. In addition, the Program contracts with the Los Angeles County (LA) to provide federal and state survey and certification services throughout that area; LA is divided into 4 District Offices.
